Each leak scores out of five, on the bands printed in Chapter 3: zero to one is solid, two to three is a moderate leak, four to five is severe. The sixth question in each set is the Volume Test. It does not change the score. Failing it raises that leak’s severity by one level, because a leak you can hold together today does not survive triple volume. The fix order is fixed: governance, then workflow, then quality. Governance first because a system executes standards and cannot invent them.
